Booking an Appointment with a Specialist
Medical services at Clinic Initio are primarily provided by prior appointment. Booking is possible through three convenient methods: By phone at 0322800815 Via the special appointment form on the clinic's website By directly contacting the registration desk If free time is available in a specialist's schedule, consultation may be possible without a prior appointment.
Patient Admission with a Specialist
Patient admission is carried out upon presentation of an identity document. For patients under the age of 18, admission requires: The identity document of one accompanying parent, and The minor's birth certificate or personal identification number. In the case of accompaniment by a relative or a third party, a notarized consent from the parent or legal representative must be presented.
Patient Pre-Notification Policy
In the event of a change to a specialist's working schedule, the clinic is responsible for informing the patient about any changes to the planned visit time or date, which is done via a call or SMS to the telephone number provided by the patient. The clinic accepts no responsibility if it is impossible to warn the patient—for instance, if the telephone is switched off, an incorrect telephone number is provided, or the patient does not answer calls.
Policy on Appointment Delays
In order to properly complete all necessary documentation, the patient should arrive at the clinic 15 minutes prior to the scheduled visit. If the patient is late for the appointment, the clinic reserves the right to change the date and time of the visit. If a patient is unable to arrive on time for the visit, they must notify the Call Center operator by phone at least 1 hour in advance.
Medical Documentation Procedures
In the case of a patient's initial visit, before the electronic outpatient medical record is opened, the patient or their legal representative must sign an informed voluntary consent for the processing of personal data and the provision of medical services. They also sign an application for receiving information via email. The patient's electronic medical record is the property of Klinika Initio and is stored on the clinic's secure server in accordance with Georgian legislation. To ensure all documentation is properly completed, it is advisable for the patient to arrive at the clinic 15 minutes prior to the scheduled visit. Should the patient or their legal representative refuse to sign the informed voluntary consent for the processing of personal data and the provision of medical services, the clinic is authorized to deny service to the patient.
Issuance of Sick Leave Certificates (Hospital Sheets)
The issuance or extension of an initial sick leave certificate (hospital sheet) for an incapacitated patient begins on the day the patient personally presents to the clinic. An exception is made if documentation of an emergency medical service (EMS) call exists. In this case, the patient must contact the clinic no later than the day following the EMS call. Accordingly, the sick leave certificate will be issued starting from the day prior to the patient's visit to the clinic.
Policy for State Insurance Users
The clinic provides paid outpatient medical services. The clinic does not participate in the State Universal Health Insurance System for outpatient services. Patients have the option to receive free outpatient services at medical facilities that provide these services within the framework of the State Universal Health Insurance System. For inpatient care, the clinic provides free planned medical services within the scope of the State Universal Health Insurance, in accordance with the license issued to the clinic by the LEPL (Legal Entity of Public Law) Agency for Medical Regulation. When receiving such services, a contract for the provision of medical care is executed with the patient or their legal representative.
Policy for Private Insurance Users
The clinic serves as a provider for all insurance companies operating in Georgia. Patients who are insured by insurance companies operating in Georgia can receive both outpatient and planned inpatient services at the clinic, in accordance with their insurance package. Patients insured by private insurance companies or their legal representatives must sign an informed consent for the processing of personal data during their initial visit. When receiving inpatient services, a contract for the provision of medical care is executed with the patient or their legal representative.
Paid Medical Services
Patients who are not enrolled in the State Universal Health Insurance system or who do not have medical insurance packages from private insurance companies can receive planned medical services at the clinic only within the framework of paid medical services, according to the clinic's approved internal tariffs. In this instance, in addition to the standard informed consents, a contract for paid medical services is executed with the patient or their legal representative.
Patients and their accompanying persons, without exception, are permitted to be in the clinic only with shoe covers (bakil).
Entry into the clinic with animals is strictly prohibited!

The purpose of data processing by the LLC "Initio" Clinic is: the provision of medical services to patients; the creation of outpatient and/or inpatient medical documentation; diagnosis establishment; protection of a person's health and life interests; data archiving; response to complaints; obtaining reimbursement from insurance companies; entering into legal relations with the data subject; protection of the person, property, and public safety; protection of the rights of a minor; and other legitimate purposes.
The Clinic processes data based on at least one of the following grounds: the data subject has given consent; data processing is necessary for the fulfillment of an obligation assumed by an agreement concluded with the data subject or for the conclusion of an agreement at the request of the data subject; data processing is stipulated by law, by-law, or a state/local health protection program/subprogram; data processing is necessary for the Clinic to fulfill its obligations imposed by Georgian legislation; the data is publicly accessible or the data subject has made it publicly accessible;
Data processing is necessary to protect the vital interests of the data subject or another person, including for the monitoring of an epidemic and/or the prevention of its spread; data processing is necessary to protect a significant public interest; data processing is necessary to protect the significant legitimate interests of the Clinic or a third party, except when there is an overriding interest in protecting the rights of the data subject (including a minor); data processing is necessary to consider the data subject's application (to provide services to them).
Passport data (name, surname, personal/identifying number, gender, date of birth): The provision of this data is a necessary condition for the subject's registration and identification. If this data is not provided, the Clinic will not be able to render services to the subject.
Telephone number, email address, place of residence, place of employment, and position held: The registrar only obtains this data upon the subject's wish, based on the information provided by the subject, and it is not verified. The Clinic is not responsible for the accuracy of this information. The absence or inaccuracy of this data may cause delays in the provision of medical services by the Clinic, such as: medical personnel contacting the patient; inaccessibility of State Universal Insurance services; doctor home visit services; sick leave certificate issuance, and others. The subject will receive medical services not related to this data at the Clinic without obstruction.
The data recipients are the Clinic's personnel and/or the personnel of the laboratory carrying out outsourced laboratory studies based on a contract with the Clinic, or, in case of referral, the personnel of another medical institution and/or an insurance organization who have a professional connection with the provision of services to the subject.
The personal data of an individual that has become known to the Clinic is stored for the mandatory period established by law: data related to the provision of inpatient medical services is stored for a period of at least 15 years, and data related to the provision of outpatient medical services is stored for a period of at least 5 years. Alternatively, data is stored for the period necessary to achieve the purpose.
The subject has the right to: receive information about the processing of their data, familiarize themselves with their data and receive copies, request correction, updating, completion, deletion, destruction, blocking, transfer, cessation of processing, withdraw consent, and appeal the issue to the Data Protection Service/Court.
Please be informed that the data subject's (Georgian citizen/permanent resident of Georgia) personal data is recorded in the Electronic Health Record System (EHR). Through this system, this data may become accessible only to the physicians whom the subject consults for the purpose of receiving medical services. Knowing the information about previously received medical services will help the subject's treating physician manage the medical service process with quality. Access to data for other persons or for other purposes is restricted. The data subject determines the transfer status (visible or hidden) of the data in the EHR system by signing the relevant document.
Consent is given for an indefinite period. The data subject can change the access status (hide or open) to the data of individual episodes of medical service at any time by submitting a written application to the Clinic, by electronic submission to the Clinic, or through the patient's page existing in the EHR system.
